A horrifying medical scam has come to light at the Government Cancer Hospital in Bikaner, Rajasthan, where expensive life-saving medications intended for impoverished patients were systematically stolen and funneled into a black market network spanning from Bikaner to Delhi. Investigations revealed that Avelumab injections, priced at approximately ₹1,00,000 each, were extracted from their vials and replaced with a dangerous mixture of plain water and bacteria. When seized samples were submitted for international forensic and chemical evaluation, testing confirmed the complete absence of the active therapeutic ingredient. Instead, the vials contained E. coli bacteria, transforming a life-saving drug into a potentially lethal contamination risk for vulnerable cancer patients.
The Black Market Racket and How Free Cancer Drugs Were Stolen
The operation was exposed after the Delhi Drug Controller conducted raids based on targeted intelligence and intercepted illegal consignments of Avelumab injections being traded on the black market. Further investigation linked the origin of these high-value pharmaceuticals back to the Government Cancer Hospital in Bikaner. The medication had been procured and supplied by RCML, the state government procurement agency, to provide free medical care to underprivileged cancer patients. Alleged collusion between hospital insiders and supply chain operators enabled the physical removal of these ₹1 lakh injections from the facility, after which they were transported to Delhi to yield massive illegal profits.
Forensic Clues: Syringe Holes and Scratched Aluminum Caps
To establish the exact methodology of the fraud, the Delhi Drug Controller sent seized samples through the manufacturing firm to Merck Lab in Germany for specialized forensic analysis. Forensic experts in Germany identified clear physical evidence of tampering on the packaging. The lab discovered a tiny 1.2 mm puncture hole in the rubber stopper of each vial, alongside distinct scratch marks on the protective aluminum cap. Perpetrators had used fine syringes to pierce the stopper, extract the genuine Avelumab liquid, and inject a crude replacement liquid before resealing the container to make it appear unopened and authentic.
Chemical Analysis and Bacterial Contamination Findings
The detailed laboratory findings from Merck Lab in Germany revealed severe chemical deviation and biological hazards. Tests proved that the primary active ingredient responsible for combating cancer cells was at zero percent in the seized samples. Under normal standards, genuine Avelumab formulations contain 8.2 percent Mannitol and 0.092 percent Acetic Acid. However, the confiscated fake injections registered only 1.3 percent Mannitol and 0.013 percent Acetic Acid. Furthermore, tests confirmed the presence of bacterial contamination, specifically identifying E. coli inside the vials. Administering such contaminated fluids intravenously to immunocompromised patients could induce severe septic shock or fatal organ complications. Pukhraj Garg, MD of RCML, officially acknowledged the findings, confirming that the tested batches were fake and adulterated.
Infertility Medications Also Compromised in Wider Investigation
The investigation by drug authorities has broadened beyond oncology care. During secondary sampling across connected supply nodes, inspectors discovered quality breaches in several medications used for infertility diagnostic procedures and treatment. Laboratory evaluations revealed that multiple infertility drug samples were unsafe and contaminated with infection-causing bacteria. Consequently, drug control agencies are expanding their audits across regional pharmaceutical warehouses, institutional distributors, and healthcare facilities to map the full extent of compromised medical supplies.
Supply Chain Vulnerabilities and Institutional Accountability
The incident has raised critical concerns regarding accountability within RCML, the Rajasthan state entity responsible for public health drug distribution, as well as internal hospital inventory control.
