When filing Income Tax Returns for medical professionals, maintaining a case register known as Form 25 is vital. This is a mandatory requirement under the Income Tax Act, 2025, although there is no immediate requirement to furnish this specific information at the exact time of filing your ITR.

Form 25 is a novel requirement mandated by the Income Tax Department specifically for doctors. It involves maintaining meticulous, day-wise records of patients, medical services rendered, fees collected, payment modes, and the date of receipt. Under this framework, medical practitioners must also document the opening and closing stock of drugs, medicines, and consumables as of the first and last days of the financial year spanning from April to March. These records can be kept either physically or electronically, provided the system is fully operational.

Form 25 is strictly mandatory for doctors only if their total gross medical receipts exceed Rs 1.5 lakhs in any of the preceding three assessment years. According to Nehal Mota, Co-Founder and CEO of Finnovate, falling into this bracket makes maintaining day-to-day patient and receipt details under Form 25 compulsory. Practitioners across any system of medicine, which includes physicians, surgeons, dentists, pathologists, radiologists, hakims, and diverse medical professionals, are required to maintain Form No. 25.

The regulatory mandate to keep Form 25 applies starting from the financial year 2026-27. Medical professionals can begin keeping these detailed records immediately; however, Nehal Mohta emphasized that maintaining patient and receipt records for a span of 7 years starting now is mandatory.

Mohta added another critical point that Form 25 remains entirely independent of whether a doctor is subjected to a tax audit or files returns under Section 44ADA. Crossing the threshold of 1.50 lakhs rupees per year in gross receipts during any of the last 3 assessment years makes Form 25 mandatory for doctors.

The primary objective behind maintaining this documentation is to curb revenue leakage and significantly improve the audit trail associated with doctors' incomes.

The noticeable contrast between the total pool of registered doctors and the declared high-income earners forms a central part of this discussion. Estimates from early 2026 indicate that India has more than 21 lakh registered allopathic and AYUSH practitioners combined. Against this substantial base, roughly 2,500 doctors reporting taxable income exceeding Rs 1 crore appears modest, especially considering the rapid expansion of private healthcare across urban and semi-urban regions.

However, interpreting these figures requires careful consideration. Not every registered doctor is actively engaged in private clinical practice. Many serve in government positions, academic roles, research institutions, administrative duties, or are retired. Others practice in smaller towns where consultation fees remain modest, while AYUSH earnings fluctuate widely based on geography, practice model, and patient volume.
